Semaglutide vs. tirzepatide: which GLP-1 is right for you?
Both medications are remarkable. Both are expensive when not covered. Both have side effects. Here's the honest, pharmacist's-eye comparison.
Semaglutide: the proven one
Sold as Ozempic (for diabetes), Wegovy (for weight loss), and Rybelsus (oral). Average weight loss is 10–15% of body weight at one year. Once-weekly injection. Strong safety data going back nearly a decade.
Tirzepatide: the new heavyweight
Sold as Mounjaro (diabetes) and Zepbound (weight loss). It hits two receptors instead of one — GIP and GLP-1 — which usually produces faster and greater weight loss. Average is 15–22% of body weight at one year.
Side effects
Both cause nausea, constipation, occasional diarrhea, and fatigue during dose escalation. Tirzepatide tends to be a bit harder on the gut, especially at higher doses. Most side effects fade within a few weeks if dose changes are done slowly.
Cost
At Parkway, both compounded semaglutide and tirzepatide are $275/month — the same. If you have insurance coverage for Wegovy or Zepbound, we run that first.
The pharmacist's take
Start with semaglutide if you've never been on a GLP-1, if cost matters more later (more brand options), or if you want the most-studied option. Choose tirzepatide if you want maximum effectiveness, can tolerate slower dose titration, and are comfortable with a slightly newer drug. We're happy to help you decide.
